Introduction

As a care provider I really value and respect your privacy. In line with my personal values and in order to meet my legal obligation as a care provider I have formulated the following privacy policy in order to be transparent about the treatment of your personal data and to ensure your awareness of rights and obligations related thereto.

Personal Data Processing

For proper treatment, it is necessary that I, as a care provider, create a client file for you. Your client file will contain notes about you, your health status and information about the examinations performed and services provided.

Your client file may also include information from third parties that is necessary for the provision of services and that, after your explicit permission, I have requested from another healthcare provider, such as the general practitioner.

Your personal data will only be processed in accordance with the purpose for which it was provided and this purpose and type of personal data is described in this Privacy Policy. As a care provider, I am the only one who has access to the data in your client file. I have a legal obligation of professional confidentiality.

I do my best to guarantee your privacy. This means, among other things, that I:

  • handle your personal data with care,

  • ensure that unauthorized persons do not have access to your personal data.

The data from your client file can also be used for the following purposes:

  • to inform other healthcare providers, for example when the provision of care has come to an end or when a referral is made to another practitioner upon your explicit permission;

  • for observation purposes during my absence;

  • for anonymized use during peer review;

If it becomes necessary to use your personal data for another reason, I will first inform you and explicitly ask for your permission.

Client rights

Your rights in regards to your personal data are well protected in the GDPR. You have the right to:

  • access,

  • recite,

  • request deletion of your personal data,

  • restrict processing of your personal data,

  • data portability.

If desired, I will give you access to the data that I store about you. Is this information incorrect? Then you have the right to request change of the incorrect personal data.

If you believe that I am unlawfully storing data about you, you have in principle the right to have the unlawfully stored data deleted.

'Restriction of processing' means that I am not allowed to use your data if, for example, it is incorrect or you have previously objected to processing. This also applies if the stored data has been stored unlawfully.

Finally, you have the right to data portability. This means that I make your data available to you, if you want to e.g. switch to another counselor.

For more detailed information about the rights and obligations related to personal data I would like to  encourage you to explore GDPR and the content provided by the Dutch Data Protection Authority (Autoriteit Persoonsgegevens).
